Effective January 1st 2011, Stone Oak Women's Center will be charging a $25.00 no-show fee for all office appointments not canceled within 24 hours. As well, a $50.00 no-show fee will be charged for all consultations and First Line Therapy appointments not canceled within 24 hours.

Scheduling your appointment

To schedule an appointment with any of our three physicians or nurse practitioner, you may call the office at 210-614-2229. If you are currently signed up on the web portal, you may also request your appointment online.

Preparing for your appointment

Please bring the following

• Driver's License or any government approved ID
• Valid insurance card
• Written list of medications (with dosage)
• If you are having a sonogram, bring a full bladder! (at least 30 oz of water prior to your appointment)
• If you wish to fill out the forms before coming into the office, you may print the forms below and bring them with you to your visit:
  • Patient Consent Form
  • HIPAA Form
  • Medical History
  • Web Portal
  • Release of Records
